Puzzle

The detective frowned at the corpse in the dentist's chair. The cause of death was clearly the syringe on the tray, but the wounds on the arms made no sense. On the victim's left forearm, there was a jagged, white scar from years ago. On the right forearm, in the exact same location, was a fresh, bleeding incision—carved with surgical precision, yet non-fatal.
